Our new patient packet typically takes 10–15 minutes to complete. It covers your medical and dental history, insurance information, and consent for treatment. Filling it out before your appointment means your first visit can be focused on your smile rather than paperwork.
Need to transfer your records to or from another provider? Our records release form authorizes the secure transfer of your dental history. Please complete it in advance so we have time to process the request before your appointment.
